Marketplace. This Form 1095-A provides information we need to

complete Form 8962, Premium Tax Credit (PTC). 


  1. Log into HealthCare.gov 
  2. Under My Applications & Coverage, select your 2024 application — not your 2025 application.
  3. Select Tax Forms from the menu on the left.
  4. Under Your Forms 1095-A for Tax Filing.
  5. Click Download [PDF].

Use the IP PIN Retrieval Tool. If you don’t already have an account on IRS.gov, you will be asked to create an account and validate your identity.


How to get your IP PIN reissued

If you're unable to retrieve your IP PIN online, you may call the IRS at 800-908-4490 for specialized assistance, Monday-Friday, 7 a.m.-7 p.m. your local time (Alaska and Hawaii follow Pacific time), to have your IP PIN reissued. An assistor will verify your identity and mail your IP PIN to your address of record within 21 days.
